What forms of payment do you accept?

Orthopedic Specialists of SW Florida accepts cash, checks, and major credit cards (Visa, Master Card, AMEX and Discover) and CareCredit for those who qualify.

 
Do you take my insurance?
We participate with the following major carriers:
  • Medicare
  • Blue Cross Blue Shield
  • Aetna
  • Cigna
  • And most other insurance plans
Will I need to pay my co-insurance and/or deductible at the time of my visit, or will you bill me?

All co-payments, deductibles and co-insurances will be collected at the time of service. Orthopedic Specialists of SW Florida will bill you for any additional unpaid balances.

 
Besides bringing my insurance card, what other documents do I need to bring with me on my office visits?
  • Photo Identification

  • Co-payment, deductible or deposit for first visit if you have no insurance

  • Social Security Number

  • Information such as date of birth, address (mailing and residence), and two telephone numbers where you can be reached

  • List of medications being taken

  • X-rays if any

  • Translator if needed

  • Completed New Patient Information Forms (available to download under the Patient Resources tab)

At which hospitals and outpatient surgery centers do you perform surgery?
   Hospitals:
  • Lee Memorial Hospital
  • Southwest Florida Regional Medical Center

   Outpatient Surgery Centers:

  • Riverwalk Surgery Center
  • Lee Island Coast Surgery Center
  • Adult and Children's Surgery Center of SW Florida
  • Gladiolus Surgery Center
What is the importance of fellowship training?

Attending internships and residency programs are requirements of the American Board of Orthopedic Surgery (ABOS). All of our orthopedic surgeons have completed an additional year of training in a specific field of interest and expertise. This extra training is called "fellowship training".
