Patient Portals
Orthopedic Specialists of SW Florida moved to a new electronic health record system, Epic, on July 16, 2026. Because of this transition, you may need to use two different patient portals depending on when your records were created.
Not sure which portal to use?
- Visit or document dated July 16, 2026 or later? Use the Epic MyChart portal.
- Visit or document dated before July 16, 2026? Use the ModMed portal.
- Scheduling a new appointment, messaging your provider, or paying a current balance? Use Epic MyChart going forward.
- Need an old visit summary, past lab result, or historical document? Use ModMed — and download it soon, since these records are not being transferred into Epic.
Epic MyChart
Use MyChart for all appointments, messages, and records dated July 16, 2026 and after. This is our active portal going forward.
First time using MyChart? When you sign up, select Lee Health as your organization — that’s who we partner with for our electronic health record.
ModMed Patient Portal
Use ModMed to access visit notes, documents, and records dated before July 16, 2026.
Important: Records from before July 16, 2026 will not be transferred into Epic. If you need copies of any past visit notes or documents, please log in and download them as soon as possible.
You will continue to have full access to the ModMed Patient Portal. This portal will remain available while we finish closing out remaining balances from before the Epic transition. We’ll post an update here well in advance of any future change to this portal’s availability.
Frequently Asked Questions
Do I need to create a new account?
Yes. Epic MyChart is a separate system from ModMed, so you’ll need to sign up for a MyChart account if you don’t already have one. If you’ve used MyChart with another provider before, you will be able to use the same account to view your OSSWF records. If you are creating a MyChart account for the first time, be sure to select Lee Health as your organization — we receive our electronic health record through Lee Health’s Community Connect program.
Will my old records be moved into Epic?
No. Records from before July 16, 2026 will remain in ModMed only and will not be transferred into Epic. If you need copies of past visit notes, documents, or other records, please log in to the ModMed Patient Portal and download them as soon as possible.
Which portal do I use to pay my bill?
It depends on when the charge occurred. Balances from before July 16, 2026 should be paid through the ModMed Patient Portal. New copays and balances from July 16, 2026 forward can be paid through Epic MyChart.
